Announcing New Services!

As we begin our new fiscal year, I am very excited to tell you about the new programs and services Park Slope Geriatric Day Center is now offering. In July we officially launched our Geriatric Care Management program, Care Path, that was made possible by a grant from the Fan Fox Leslie R. Samuels Foundation. Care Path is a comprehensive geriatric care management service which helps elders and their loved ones cope with the challenges of aging to maximize their quality of life. This service can help caregivers navigate the health care system and help reduce the stress associated with caring for a loved one.

In response to the needs of the community and made possible by a grant from the Brookdale Foundation Group, we implemented an Early Memory Loss program, The Lotus Group, specifically designed for individuals experiencing early signs of dementia. The group meets on Tuesday’s and Thursdays and offers specialized programming to help strengthen memory, increase socialization, and provide peer to peer support. We have already received positive feedback from the participants and their caregivers. We hope to offer the program five days a week later this year.

In September, we began offering memory screenings in collaboration with Dr. Goldenberg and associates at the Memory Center at New York Methodist Hospital. These screenings test individuals for cognitive impairment. We all know that early detection is key to successful treatment, improving one’s quality of life, and reducing caregiver stress. The new Caregiver Education Series meets on Thursday afternoons at Leeza’s Place. Through this program we will continue to offer vital information on caregiving, aging, health, and other related issues with presentations from our Caregiver Associates and guest speakers.

As we have for the last 23 years, we strive to meet the needs of our clients and surrounding community. Care Path is a holistic service. Our senior care manager works with you to look at physical, medical, and emotional concerns, as well as financial and legal issues that will determine the best approach. We’ll link you with necessary services, whether help to stay at home as long as possible, or seek other appropriate housing, determine benefit eligibility, find the right doctor, or help you think through possible short and long-range scenarios.
